In order for the Perpetual Rehabilitation Fund to consider a sponsorship, it is required that you give us a little information first. We realize that paper is the last you want to do and that you would just like to get the help you need and get it now, with this in mind we have tried to make the process as painless as possible. We recommend that after you download the application you find a quiet place away from others and if possible consult with a close friend, your pastor or bishop or possibly a sponsor from a 12 step group who would be non-judgemental and be able to assist you in expressing your thoughts and feelings on paper.
